An unannounced renewal visit was conducted today 09/09/2015. There were 11 children in care at a total of 34 points with the licensee and three assistants. The Inspector arrived at 10:15 am and departed the program at 1:00 pm. The inspector reviewed five children's records, the licensee's, three assistant's and one household member's record. Observed the children during: free play, dancing, story time, lunch and nap. Discussion with the licensee about: record clean up in household member and staff records, relocating the violation notice, using the model form for the emergency preparedness plan and outside play.

Standard #: 22VAC40-111-800-B
Description: Based on review of the emergency preparedness plan, the licensee failed to annually review plan. Evidence: 1. The inspector reviewed the plan and found that it was last updated on 10/15/2013.

Plan of Correction: The licensee updated the plan during the inspection and will monitor annually.

Standard #: 22VAC40-111-120-A
Description: Based on review of three staff records, the center failed to ensure that all records were complete. Evidence: 1. The inspector found that staff # 1 and 2 did not have verification of age.

Plan of Correction: The licensee will have the staff to bring in documentation for verification of age.

Standard #: 22VAC40-111-30-C*
Description: Based on observation and staff's record review the licensee failed to ensure that all staff had a returned background check prior to being alone with children. Evidence: 1. The inspector found that staff # 1 was hired on 9/1/2015 did not have a returned CRC check and was alone with three children. The licensee was in another room with additional children children.

Plan of Correction: The licensee will change the schedule of staff to ensure that all staff that are alone with children are with a staff person that has a returned CRC check.

Standard #: 22VAC40-191-60-C-1
Description: Based on review of four staff records, the licensee failed to ensure that all CRC checks were returned. Evidence:s 1. The inspector found that staff # 2 did not have a returned CRC check with in 30 days of hire. The CRC was returned on 6/24/2015 and the hire date was 4/8/2015.

Plan of Correction: In the future the licensee will document when the CRC's are returned for any reason and will monitor.
