Technical Assistance:
To learn more about new on-line classes for providers from the state, go to the website at and under children and child care, look for provider resources and training opportunities. Consultation today included physical environment, licensing standards regarding diapering surfaces and record keeping.

An unannounced monitoring inspection took place today from 11:45 - 1:30 pm. There were 11 children present for 40 points with the provider and two assistants. I observed playtime with blocks and large legos; preparation for lunch; mealtime and preparation for nap. Outdoor areas were not inspected today. If you have any questions, please contact me at

Standard #: 22VAC40-111-830-A
Description: A review of records today and staff confirmation found that emergency evacuation drills have not been practiced monthly as required. Evidence: There was no documentation of fire drills since May 2015.

Plan of Correction: We will practice two a month to catch up.

Description: During today's inspection, it was observed that not all of the children's infant bottles were labeled with their name and the date.

Plan of Correction: We will get them labeled.

Description: A review of records today found that one child did not have documentation of their physical exam within 30 days of attendance.. Evidence: Child # 4, who began care 6/15, did not have a physical exam accessible today.

Plan of Correction: I will talk to the parent today.

Description: A review of staff records today found that one assistant did not have all required information. Evidence: There was no emergency contact information and documentation of education and experience as required for Assistant # 2.

Description: A review of children's records today found that some information was missing. Evidence: Child #1 had no documentation of a review and annual update of her record (due 3/15); Child #2 did not have a signed authorization for emergency treatment and addresses of two emergency contacts. Child # 5 had incomplete health insurance information and Child # 6 was missing both emergency contacts' phone numbers.

Description: During a review of children's files today, it was found that not all children had documentation of proof of identity as required. Evidence: There was no information completed to document receipt of proof of identification for Child # 3 and #4. ,

Description: During today's inspection, a child was observed in attendance before the parents submitted the required proof of adequate immunizations. Evidence: Child # 1, who was present today, did not have documentation of her immunizations available.

Plan of Correction: I will talk to the mother today.

Description: During today's inspection, a piece of equipment was observed to not be used in accordance with the manufacturer's instructions. Evidence: Two infants were observed to be sleeping in bouncy seats that had been placed on the mattresses inside two cribs. (Bouncy seats are meant to be placed on stable surfaces.)

Description: During today's inspection, two children were observed to be in high chairs at lunch without the belts fastened.

Plan of Correction: I will remind my assistant.

Standard #: 22VAC40-111-680-A
Description: During today's inspection, a caregiver was observed to not wash hands after diapering a child. Evidence: A caregiver was observed diapering two children in a row without first washing hands between them.

Plan of Correction: She will remember to wash after each child.

Description: During today's inspection, the diapering surface was not observed to be cleaned and sanitized after each use as required. Evidence: Today the diapering surface was wiped down with disinfectant wipes, but not sanitized with an approved sanitizing spray afterwards.

Plan of Correction: We will use the Clorox mixture to make a sanitizing sanitizer.
