The licensing inspector conducted an unannounced renewal inspection from 9:25am - 11:10am. There were three children in care for a total of ten points with two caregivers. The children were observed at free play and watching cartoons. Three adult and two children's records were reviewed. Violations from the previous inspection were also reviewed and two were repeated. Standard #: 22VAC40-111-70-A
Description: Based on record reviews, the family day home failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed two children's records. One of two records reviewed (child record #1) did not contain the policy number for the child's medical insurance.

Plan of Correction: The provider has already requested this information, but will request it again. The provider will review the children's records quarterly to ensure all information is complete.

Standard #: 22VAC40-111-350-E
Description: Based on observation, the family day home failed to maintain the hot water temperature within the acceptable range of 105 to 120 degrees. Evidence: 1. The inspector used the thermometer to measure the hot water temperature. The hot water temperature measured 129 degrees.

Plan of Correction: The provider has already turned down the hot water heater, but this has not helped. The provider will purchase a scald guard and place on the sink. The children will be monitored closely while hand washing until the scald guard is installed.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record reviews, the family day home failed to obtain repeat background checks. Evidence: 1. The inspector reviewed three adult records. Two of three records reviewed (adult record #1 and #2) contained criminal history record checks that were more than three years old. Adult record #1's criminal history record check was dated 05/30/12 and adult record #2's criminal history record check was dated 05/21/12. The sworn disclosure statements and the searches of the central registry were current.

Plan of Correction: The provider has already requested repeat criminal history record checks for adult records #1 and #2, but to date has not received them. The provider will contact the Virginia State Police Central Criminal Records Exchange and inquire about the status of these background checks. The provider will review the adult records quarterly to ensure all information is up to date.
