Comments:
An unannounced monitoring inspection was conducted on September 30, 2015 from approximately 9:00 AM to 10:10 AM. Upon arrival the provider and assistant were present with 5 children (15 points). Children were observed watching TV, freely playing and napping. Two children's records, one caregiver record and pet records were reviewed. The exit interview was conducted with the provider.

Violations:
Standard #: 22VAC40-111-770
Description: Based on observation, the provider did not have a working portable battery-operated weather band radio and extra batteries in the home. During the inspection conducted on September 30, 2015, the weather battery operated radio was not working.

Plan of Correction: Provider will get new batteries.
