Conducted an unannounced renewal study with the provider and one child, age three years-old, for a total of two points. The child was coloring pictures, playing on the computer, and having free play with toys during the inspection. One child's record and three household records were reviewed. The provider is current in First Aid and CPR. Start time: 9:00am End time: 11:55am
Standard #: 22VAC40-111-100-A Description: Based on a review of one child's record, the record was missing a physical examination. Evidence: Child A with an enrollment date of 8/18/2014, was missing documentation of a physical examination, which was due within 30 days after the first day of attendance. Plan of Correction: The provider stated that he will give the forms to have the parent's get one completed again.
Standard #: 22VAC40-111-60-B Description: *Repeat Based on a review of one child's record, the record was incomplete. Evidence: The name, address, and telephone number of the child's physician was missing from the record. Plan of Correction: The provider stated that he will get this information completed by today.
