- Technical Assistance:
The provider asked about a family member who the provider may use as an assistant. The requirements for an assistant were reviewed including background checks, TB test, orientation, first aid and CPR (complete caregiver record), education and experience (Standard 140-B-2). The provider asked about how to get in touch with the agencies that are doing the background checks. Phone numbers were left with the provider for the VA State Police and DSS central registry contacts.
An unannounced renewal inspection was completed on 10/9/15. There were three children in care between eight months and two years old with the provider supervising. The point total was 10. Ten children's records and the provider's record were reviewed. The provider will need three and one-half more hours of training before the end of the calendar year. An upcoming training in the area was explained to the provider and information was given to the provider on all details of this training. The inspector arrived at the provider's home at 9:15 am and departed at 2:30 pm. The children were observed during breakfast, during free play time, during lunch, during story times, during music (singing time), during nap time, and during departure of one part day child. All infant activities were observed. There was discussion with the provider about the following: getting immunization updates for children #4 and #7, having documentation of a physical examination by 10/14/15 for child #9, checking forms for consistency in policies (medication and liability insurance), recommendation of having the "Authorization to Apply a Non-Prescription Topical Skin Product" form completed by each parent for a homeopathic spray that may be used for insect repellent, upcoming updates of children's records and emergency preparedness and response plan, nighttime practices, routines and supplies for one child who attends after 7:00 pm, ways to keep up with updates (spreadsheet with expirations listed that is posted in a visible area for the provider to see regularly).
Standard #: 22VAC40-111-60-B Description: Based on record review, the provider failed to ensure that six of ten children's records contained complete information that is required by the Standards. Evidence: 1. The record of child #2 was missing the form "Information for Parents" and "Policy for Administration of Medications." 2. The record of child #6 was missing a policy number for the child's medical insurance. This child was also missing documentation of a complete address for one of two of the child's emergency contacts (street address and city only - no house number). 3. The record of child #7 and #8 were missing the school and grade level of a school that the children are concurrently attending. 4. The record of child #9 was missing the work address for the father. 5. The record of child #10 was missing parent confirmation (not signed by the parent) of receipt of the "Policy for Administration of Medications" form. Plan of Correction: The provider will get all of the information and forms completed. In the future, the provider will make sure that all forms and all information is filled out completely when the child records are completed at enrollment or at each annual review.
Standard #: 22VAC40-111-600-B Description: Based on observation, the provider failed to ensure that infants and toddlers shall did not spend more than 30 minutes of consecutive time during waking hours, with the exception of mealtimes, confined in a crib, play pen, high chair or other confining piece of equipment. The intervening time period between confinements shall be at least one hour. Evidence: 1. There was an infant in care, eight months old, observed in a high chair eating breakfast at arrival of the inspector (9:15 am). The infant was taken from the high chair after eating breakfast and placed directly in an infant swing (approximately 9:30 am). The infant was awake when put into the swing and awake the entire time while in the swing. The provider took the infant out of the swing and placed him directly into a play pen in the playroom. The infant was observed awake in the play pen. The infant fell asleep in the play pen at approximately 10:40 am. The provider removed the infant from the play pen at 10:45 am. The infant woke up when the provider picked him up out of the play pen. The provider placed the awake infant in an exersaucer directly from removing him from the play pen. The infant stayed in the exersaucer until approximately 11:00 am when the provider removed the infant to feed him. The infant was held by the provider from this time (11:00 am) until 11:20 am while the infant was being fed. The infant was placed on the floor to play after this until 11:50 am when the infant was placed in a high chair for lunch. Plan of Correction: The provider will make sure that any infant is not in a confining piece of equipment while awake more than 30 minutes and is out of any equipment for at least one hour.
Standard #: 22VAC40-111-680-B Description: Based on observation, the provider failed to ensure that the children's hands were washed as required. Evidence: 1. A two year old child was observed arriving at the family day home. The child was placed in a high chair by the provider directly after arrival. The child was given breakfast and the child did not have her hands washed prior to eating. Plan of Correction: The provider will ensure that children's hands are washed at arrival if they are eating breakfast and as required.
Standard #: 22VAC40-111-690-D Description: Based on observation, the provider failed to ensure that the diapering surface was cleaned and sanitized after each use. Evidence: 1. The provider was observed during three diaper changes. After two of the diaper changes, the provider was observed spraying the diaper surface with a bleach and water mixture and wiping it dry immediately. The diaper surface was not sanitized. The provider was asked about the normal procedure and the provider stated that the diaper surface is sprayed with bleach and water and wiped dry (just as what was observed). Plan of Correction: After the second diaper change, the procedure was explained to the provider. After the third diaper change, the provider cleaned and sanitized the diaper surface as required. The provider will ensure that the surface is cleaned and sanitized after each change in the future.
Standard #: 22VAC40-191-40-D-1-c Description: Based on record review, the provider failed to ensure that a background check was completed at least every three years as required. Background checks are required before three years since the dates of the last sworn statement or affirmation, most recent central registry finding and most recent criminal history record check report. Evidence: 1. The last completed Criminal Record Check (CRC) for the provider was dated 8/6/12. The last completed DSS search of the central registry was dated 8/15/12. 2. The provider requested assistance in completing the forms from the licensing inspector on August 18, 2015. There were multiple conversations with the provider on August 20, 2015 about how to complete the forms. Plan of Correction: The checks have been sent. In the future the provider will make sure the checks are sent off before expiration in time to get them returned before three years of the last completed.
