An unannounced renewal inspection was conducted October 15, 2015 from approximately 10:00am to 12:30pm to determine the provider's compliance with licensing standards. In attendance were nine non-resident children. The children were being supervised by two caregivers for a total of 26 points. During the inspection, children were observed playing outside, playing with toys, creating ?Cat in the Hat? hats, reviewing rhyming words, singing songs and helping clean up their toys. Lunch consisted of tuna fish and whole wheat crackers, green beans, hash browns and milk. The provider?s home has five primary rooms for the children to use along with a bathroom. A complete walk through of the home was conducted with the provider. The outside play area was also inspected. During the inspection the emergency supplies were checked including the first aid kit, flashlight, and weather band radio. Documented practiced emergency drills, emergency preparedness plan, daily attendance and staff training hours were also reviewed during the inspection. The provider stated medication is not administered at this time. Transportation is not provided. Three children?s and three caregivers? records were reviewed during the inspection. The risk profile tool will be used for the licensing recommendation once all of the necessary paperwork is received by the licensing department. The renewal application was received prior to the expiration of the current license. Until the new license is received, the provider is legally operating. Standard #: 22VAC40-111-120-B Description: Based on review of three caregiver records, one record did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Caregiver #2 (hire date 10/12/15) did not contain the name address and telephone number of a person to be notified in an emergency and documentation that two or more references as to character and reputation of the caregiver were checked before beginning employment. Plan of Correction: Per provider - I will add the information to the record for Staff #2.
Standard #: 22VAC40-111-60-B Description: Based on review of three children's records, one record did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Child #3 (enrollment date 03/03/15) did not contain a name, address, and phone number for two emergency contacts. One name, address and phone number was listed in the record. Plan of Correction: Per provider - I will obtain the additional information and add to the child's record.
Standard #: 63.2-1720-F Description: Based on observation and interview, the provider did not ensure an employee without an completed original criminal clearance did not have direct contact with children without the required supervision. Evidence: During the inspection, the licensing inspector observed Caregiver #2 (hire date 10/12/15) supervising five children on the outdoor deck. Caregiver #1 was supervising four children inside the home. The two groups were separated for approximately 45 minutes. The record for Caregiver #2 did not contain a completed criminal history record. Caregiver #1 stated there were typically three caregivers working throughout the day. An additional caregiver with a completed background check typically worked with Caregiver #2 but the third caregiver was out sick for the day. Caregivers are not permitted to work in a position that involves direct contact with a child receiving services until an original criminal record clearance or original criminal history record has been received, unless the caregiver remains within sight and sound / direct supervision of another caregiver for whom a background check has been completed. Plan of Correction: Per provider - Typically Caregiver #2 is not working alone. The additional caregiver had called out sick for the day. I will ensure Caregiver #2 is supervised by other caregivers until the required background checks are obtained.
