An unannounced renewal inspection of the facility and records was conducted from 9:00am ? 10:35am. There were 5 children present equaling 12 points with the provider. Six children?s records and one household record and one assistant record was reviewed. Children were observed in transitions, transportation, provider lead and child lead activities. The inspector reviewed the posted license, inspection results, menu, and fire drill documentation. A walking tour of the outside play space was made as a part of the inspection. The provider reports that there is not any medication being administered. The following violations were observed and will appear on the violation notice: lack of documentation of proof of identification and lack of updated background check information.

Standard #: 22VAC40-111-60-B
Description: Based on record review an interview the provider failed to ensure that each child's record contains documentation of proof of identity. Evidence: 1 - The records for children 1, 2, and 3 do not have documentation of proof identification. - Child 1 has a documented first date of attendance as 2/10/15. - Child 2 has a documented first date of attendance as 2/10/15. - Child 3 has a documented first date of attendance as 6/17/14.

Plan of Correction: The provider will ensure proof of identity is viewed and documented in each child's record within the time frames allowed.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review and interview the provider failed to ensure that Sworn Disclosures are updated within three years as required. Evidenced; The record for the licensee contains a Sworn Disclosure and Affirmation statement dated 10/18/12. Therefore, the background check has expired.

Plan of Correction: Corrected during the inspection. The provider will create and implement a record keeping system that will alert her as to when an expiration date is approaching.
