The provider is present with 3 children totalling 8 points. At the beginning of the visit the children are napping after the nap the children were playing and drawing. Interaction between provider and children observed. The provider has lots of supplies for the children to play with and a reading area and a room for lots of play. Standard #: 22VAC40-111-60-B Description: The provider failed to get some information that is required by the standards. Evidence: Based on the review of 3 children's records, two children are missing some information. Child A and B are both missing physicals, they have both been at the providers more than 30 days. Child A also is missing teh physicians information and a second emergency contact. Plan of Correction: I will ask the parents for the information.
