Comments:
The licensing inspector conducted an unannounced monitoring inspection, at which play activities were observed. The child was observed; and staff and children files and other documentation were reviewed. One (1) child was found to be in care totaling 4 points. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 11:05am and concluded at 12:20pm.

Violations:
Standard #: 22VAC40-111-830-A
Description: Based on review of documents and discussions with the provider compliance was not met as required relating to monthly emergency evacuation drills. Evidence: There had been no practiced emergency evacuation drill conducted for June 2015 as required. There had only been a practiced shelter-in-place drill conducted for that month. Emergency escape drills are to be conducted and recorded monthly; shelter-in-place drills are to be conducted 2 times per year and recorded.

Plan of Correction: All drills will be conducted and recorded as required for continuing compliance.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on review of files and discussions with the provider there was not compliance with renewal of background checks as required. Evidence: The provider background checks had expired as of 10/24/15; no renewal checks were available. Renewal requests for background checks had been sent off on 11/6/15 according to the provider. Background checks are to be renewed every 3 years, prior to expiration of the original background checks.

Plan of Correction: Requests for new background checks were sent off on 11/6/15. In the future all requests for renewal of these will be sent off 30 days prior to expiration of the ones on hand to ensure compliance.
