54.1-3408 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
Provider and household member need the CRC checks to be renewed before January, 2016. Discussed yearly checks of enrollment forms- parents review, sign and date that paperwork is still current.

An unannounced inspection has been conducted today. There were four children present with the Provider and an assistant for a total of 10 supervision points. Provider has paper work maintained in organized file and is currently resubmitting for CDA renewal. The Provider's MAT, CPR, and First Aid are current. The home has materials for the children that are age/stage appropriate for the children and has set up area for children on the main floor. The Inspector reviewed staff records, children's records, special care (emergencies), and paperwork pertaining to zoning approval. Please call with questions 703-479-4679

Standard #: 22VAC40-111-210-A
Description: Based on record review, the assistant has not obtained 16 hours of annual training based. Evidence: 1. Assistant started in October and training certificates between that time totaled 13 hours.

Plan of Correction: I will have her attend 16 hours as I though she was on my time frame and I am current in training.

Standard #: 22VAC40-111-530-G
Description: Based on observation, the Provider is allowing children under two years of age to sleep with pillows. Evidence: 1. Four children observed sleeping and two children were 20 months old. Both were sleeping with a pillow.

Plan of Correction: I will take the pillows away.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review, the Provider has not obtained central registry checks before three years since last one for herself and a household member. Evidence: 1. CPS checks were conducted in November, 2012.

Plan of Correction: I will send them out.
