Nine children were in care during the inspection. The age range of the children was 12 months to 13 years old. Total points were ten. The provider and one assistant were present. Children were resting then free play. No children were on medications. Inspection time 1:45pm-4:00pm
Standard #: 22VAC40-111-60-A Description: Based on review not all children's records were complete. Evidence: Child A with a start date of 7/1/13 did not have one emergency contact and was missing mother's work number. Child B with a start date of 8/17/15 was missing work address for mother, the insurance policy number and one emergency contact. Plan of Correction: I will ask the parents for the missing information.
Standard #: 22VAC40-111-210-A Description: Based on review the 16 hours of annual training was not completed. Evidence: The provider only competed 6.5 hours of annual training and Assistant A completed 7.0 hours of annual training. Plan of Correction: We will attend the required training this year and make up the missing hours.
