Technical Assistance:
Discussed with provider: Emergency preparedness relocation site, Crib mattress recall, Weather band radio recalled items, gstes for stairs. Form SP-230 was provided for adults.

An announced Initial study was conducted today. There are currently no children enrolled in the day care. Your application for Licensure cannot be submitted for approval until the completed search results are on file. Please contact Richmond to verify that the searches were at least received in that office. Additionally, you must be current in First aid and CPR certification. The crib assembled for use has an acceptable manufacture date of year 2015, week 35. Please contact me as soon as your search results and first aid certification are current. I Will need to view the original documents prior to submitting your application for approval. LI was present today from 8:50 AM - 10:25 AM. Description: Based upon a review of first aid supplies, the first aid kit is incomplete. Evidence: There is not digital thermometer available.

Plan of Correction: I will purhace a new digital thermometer.

Description: The weather band radio is not battery operated. It is a hand crank and USB (electric power) only.

Plan of Correction: I will replace the radio with a battery operated one.
