Comments:
Today an unannounced renewal inspection was conducted with the Provider-Mrs. Puza. This visit took place in the morning between 9:00-11;15 there were 6 children with the Provider and 1 assistant for a total of 12 points within the supervision guidelines. The children were observed having morning breakfast, snack singing songs, having circle time and learning about different animals and sounds. Breakfast consisted of oatmeal, hard boil eggs and milk. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, menu and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-111-750-A
Description: Based on observation of the physical plant, topical skin products were found accessible to children. Evidence: In the bathroom, diaper ointments were found on a shelf accessible to the children.

Plan of Correction: Provider stated that she will move them.

Standard #: 22VAC40-111-180-A
Description: Based on observation of the caregiver records, one record did not include documentation of an initial TB screening. Evidence: Caregiver #1's record with a start date of 9/1/15 did not have documentation of an updated TB screening (11/13)

Plan of Correction: Provider stated that she will have her get a new Tb screening immediately.

Standard #: 22VAC40-191-40-D-1-b
Description: Based on observation of the caregiver records, one record did not include documentation of a Sworn Disclosure statement prior to their first day of employment. Evidence: Caregiver #1's record did not include documentation of Sworn disclosure statement (9/1/15 start date)

Plan of Correction: Provider had them complete the form during this inspection. Provider stated she was not aware she needed to fill it out.
