Comments:
An unannounced monitoring inspection was conducted December 10, 2015 from approximately 9:30am to 11:20am to monitor the provider?s probationary status per the Special Order issued in September 2015. There were eight non-resident children and two resident children present during the inspection. Two caregivers were supervising the children for a total of 25 points. Children were observed having their diapers changed, taking bathroom breaks, eating, coloring pictures of Christmas ornaments, playing with toys, and playing outside. Lunch consisted of meatballs and flatbread, beets, tater tots and milk. During the inspection the menu, daily attendance, and required postings were checked. All were found to be in compliance. The provider stated medication is not administered at this time. Transportation is not provided.

Children and caregiver records were not reviewed during the inspection as there has not been any change in caregivers or new enrollments. If you have any questions regarding the inspection, contact the licensing inspector, Lynn Powers at (804) 662-9790.

Description: Based on observation, the provider did not ensure all furnishings were kept in a safe and operable condition.

Evidence:
During the inspection, the licensing inspector observed a wooden picnic setting outside in the fenced in outdoor play area. The wooded boards which makes up the seating area of the picnic table was deteriorating and contained protruding nails. The deteriorating wood and nails could entangle or snag skin.

Plan of Correction: Per provider - We will repair and replace the worn areas of the picnic table.
