An announced Initial study was conducted today. There are 4 children (8 points) and 2 adults present. 4 adult and 4 children's files were reviewed. See violation notice form missing information. The two cribs have manufacture dates of 2013. Children were observed in a variety of activities today. Menu, first aid supplies, emergency supplies, emergency evacuation drills are all in compliance. Standard #: 22VAC40-111-120-D Description: Based upon a review of files, one adult file is incomplete. Evidence: Adult B has not documentation of a current TB test/screening on file. (Date of Hire: 1/15/2016). Plan of Correction: I will have the test done as soon as possible.
Standard #: 22VAC40-111-70-A Description: Based upon a review of files, one child's file is incomplete. Evidence: Child 1's file is missing acknowledgement of receipt of the Liability Insurance declaration. Plan of Correction: I will complete the form and ask the parent to sign the form.
