54.1-3408 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
Medication Administration Training (MAT) certification will expire April 2016 Please contact Piedmont Licensing Office prior to submitting (via mail) Renewal Application to ensure proper delivery.

Comments:
An unannounced monitor inspection was conducted on 1/20/2016. There were a three (3) children and the Provider present during the inspection. Children present totaled five (5) points under the direct supervision of 1 caregiver. Records for recently enrolled children & staff/household records that were previous found to be out of compliance during the previous inspection were reviewed. The Provider is currently certified to administer medication however there were no children present receiving medication during the hours of operation. Active participation in the Child Care Food Program and other county/state sponsored programs continue to be a part of the services offered to enrolled children & their families. Daily activities for the children include snacks/meals, quiet/independent activities, directed/group activities & outdoor play (weather permitting). Areas of non-compliance & Risk Assessment were reviewed with the Provider during today?s inspection 1/20/2016.

Standard #: 22VAC40-111-450-A
Description: Based on observations, the Licensee failed to clearly mark doors with clear glass panels that reach within 18 inches of the floor with decorative objects or decals at the eye level of children in care., EVIDENCE: The full glass front storm door was not marked with decorative objects or decals.

Plan of Correction: I changed it while LI Anderson was here.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review, the Licensee failed to update Background Checks (Criminal Record) before 3 years since the dates of the most recent Criminal Record Check report. EVIDENCE: Record for Staff/household member #2 did not include documentation of a updated Criminal Record Check (3). Most recent CRC was completed 7/13/2012 and was not updated before 3 years.

Plan of Correction: I am going to fix it and have it in the mail by Monday
