Discussed with provider options to maintain required ratios during nap time if provider needs to leave the premises to pick up children from the bus stop.
An unannounced renewal inspection was conducted on 01/07/2016.There were 8 children (18points) observed with the provider and one assistant. The children were observed during nap time and participating in quiet activities until everyone was ready to get up. The following areas were inspected during the visit: physical environment, posted documents, records (children, caregiver, and household), emergency preparedness plan, first aid kit, emergency supplies, menu, emergency drill log, and attendance log. Violations cited during the inspection can be found on the violation notice. An exit interview was conducted at the conclusion of the inspection. Standard #: 22VAC40-111-830-D Description: Based upon records reviewed, documentation was incomplete or missing. Evidence: There was no documentation of a shelter in drill having been completed for the month of November 2015. Plan of Correction: Provider will remind assistant to document all required drills.
Standard #: 22VAC40-111-60-B Description: Based upon records reviewed, required documentation was found to be incomplete or missing. Evidence: Child A's record is missing child's physician information, insurance policy number and 2 of 2 required emergency contacts were not provided. Child B's record is missing insurance policy number and complete information for emergency contact #2 Plan of Correction: Provider will request information from parents at pick up time.
Standard #: 22VAC40-191-40-D-1-c Description: Based upon records reviewed, required documentation was found to be incomplete or missing. Evidence: Assistant #1's record contained a sworn statement , central registry findings, and criminal history record check report that all expired 09/2015. Plan of Correction: Provider was in the process of mailing renewals when the inspector arrived.
