Technical Assistance:
Required records for back-up or substitute caregivers was reviewed with the provider. Files must be kept and available for any person used as a back-up or substitute caregiver.

Comments:
An unannounced mandated renewal inspection was conducted today. Eight children, ages, five months - four years, totaling twenty-three points, were in care with two caregivers. The inspector arrived at 9:15 Am and departed at 11:30 AM. Morning activities and snack service were observed. A sampling of four children's records were reviewed. First aid/CPR and medication administration training of caregivers were updated. Emergency drills and injury records were reviewed. The provider reported that no medication has been administered to children in care. An exit meeting was conducted with the provider, during which the provider's plans of correction for the cited violations in the area of administration were obtained. Please contact the inspector if there are any questions.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based upon review of four children's records, the provider has not ensured that each child's records contains the addresses for the two persons designated to be contacted in an emergency if the parent cannot be reached. Evidence: The records for child 2 and child 3 do not have dcoumented the addresses for the two persons designated to be contcted in an emergency.

Plan of Correction: The parent of these children will be contacted to supply the street addresses for the emergency contact persons.

Standard #: 22VAC40-111-70-A
Description: Based upon review of four children's records, the provider has not ensured that before the child's first day of attendance, the parents are provided in writing the required information. Evidence: 1. The parent information forms for children 2 and 3 were signed by the parent, however the information on the forms had not been completed by the provider. The following information was not provided: check in check out procedures, the general daily schedules appropriate for the ages of the children; use of a substitute provider, whether or not time out is used and the policy for termination of care. According to the records, children 2 and 3, both in care during the inspection, began attending 11/9/15. 2. The record for child 4, who was in attendance during the inspection and who, according to the record began attending on 12/18/15 does not include providers decision to administer medication form.

Plan of Correction: The information was completed on the parent information forms for children 2 and 3 and will be reviewed with the parent upon pick-up today. The decision to administer medication form will be completed and reviewed and signed by the parent at pick-up today.
