An announced initial study was conducted with provider. There was one day care child present totalling two points. The child was playing with age appropriate toys. Provider has a lot of age appropriate toys, puzzles, books, and supplies. Provider will not administer medication - Medication Administration Training is required to administer medication. First aid and CPR certification is valid until 3/3/2017. Pet immunizations are complete. There is no fireplace in the home. One child's file and two household files were reviewed and are complete. Emergency Preparedness and Response plan was reviewed. Lunch served today: chicken nuggets, corn, apples, and milk. Description: Based on observation, the provider did not have a complete first aid kit. Evidence: there were not two triangular bandages in the first aid kit.

Plan of Correction: Provider will purchase or make two triangular bandages for the first aid kit.

Standard #: 22VAC40-111-240-A
Description: Based on observation and review of five cribs, the provider had one crib that does not meet federal regulations. Evidence: one crib in the baby room had a manufacture date of 5/2009. There was no documentation to support that the crib meets the Consumer Product Safety Commission regulations effective 6/28/2012.

Plan of Correction: Provider will remove the crib that does not meet regulations.
