Comments:
Today an unannounced monitoring visit was conducted with the Provider & 1 assistant. This visit took place in the morning between 9:30-1. There were 10 children present including the Provider son with a total of 27 points within the supervision guidelines. The children were observed working on fine & gross motor skills, having free play, cutting paper, lunch and nap time. Lunch today consisted of pasta vegetable soup w/beans, fruit and milk and for snack homemade blueberry muffins. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-540-A
Description: Based on observation of the infants, a swing occupied by an infant was not being used according to the manufacturer's instructions. Evidence: Upon arrival, an infant was found in a swing sleeping.

Plan of Correction: Provider stated that the child has acid reflux, and the parent request that the child be placed in a incline position. Provider will discuss with the parents and request that a doctor's note is provided requesting for the child to be placed on an incline.
