Today an uannnounced renewal inspection was conducted with the Provider. This visit took place in the afternoon from 12-2 there were 8 children present with the Provider and 2 assistants for a total of 28 points within the supervision guidelines. The children were observed having free play, drawing, blocks, watching Tv while the Provider was in transition for lunch and nap time. Lunch today consisted of beef and vegetable soup, fruit and milk. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-920 Description: Based on observation and interview with Provider, the provider did not have documentation of a menu for the current one-week period posted. Evidence: No menu posted for the current week 2/8-12. Plan of Correction: Provider stated that she normally has the menu posted on the board however she painted the wall and removed the board. Provider stated that her husband will hang the board today.
Standard #: 22VAC40-111-120-B Description: Based on record review, one caregiver record (#7) is missing documentation that two references were checked prior to their first day of employment. Evidence: Caregiver #7 began employment on 9/8/15. There is no documentation of reference checks in their file. Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 22VAC40-111-60-B Description: Based on record review, 2 children's records (#4 and #7) are missing complete required documentation in their files. Evidence: Both children's records #4 and 7 are missing addresses for both emergency contacts listed. Child #4, who has a start date of 5/11/15 has no documentation of a physical examination or updated immunization records in their file. Plan of Correction: Provider stated that the parent was waiting for the child's one year old appointment. Provider notified the parent during this insepction and is waiting for the parent to fax the physical and immunization.
Standard #: 22VAC40-111-410 Description: Based on observation, a portable heater did not include a mesh guard and was not securely mounted out of the reach of children. Evidence: Upon arrival, a portable heater was found on the floor accessible to children. During the course of the inspection, the children were observed playing with the buttons. Plan of Correction: Provider stated that she will remove it immediately and make sure that it is inaccessible to the children at all times. Provider stated that they do not use it since she has a mounted heater.
Standard #: 22VAC40-111-530-F Description: Based on observation, soft bedding was used around infants in the cribs. Evidence: Two cribs were observed with stuffed animals, pillows and quilts in them during this inspection. Plan of Correction: Provider immediately removed the stuffed toys and comforters/blankets from the cribs during this inspection.
Standard #: 22VAC40-111-540-A Description: Based on observation, children were observed in feeding tables/high chairs without safety strap securely fastened. Evidence: 3 toddlers were observed in feeding tables/high chairs however they did not have a safety strap fastened. Plan of Correction: Provider strapped all of the children in during this inspection.
