Description: Based on observation and information from the children and the licensee, the licensee did not follow the nutritional guidelines of the United States Department of Agriculture. Evidence - There was no milk provided with lunch. There was only one of two required fruit or vegetable components provided for lunch. There was only one of two required food components provided for snack.

Description: Based on a review of children's records, the licensee does not ensure that the home's services are conducive to the welfare of children in care. Evidence - The record for child #9 contains information from both the child's parent and the child's doctor stating that the child has a "severe" allergy to nuts. The parent states in writing that the child should be given an antihistamine if an exposure occurs, however, there is no antihistamine available on site.

Description: Based on a review of records of family members, the licensee did not obtain a new tuberculosis screening for one family member every two years. Evidence - The record for family member #6 has a tuberculosis screening that expired on January 17, 2016.

Description: Based on observation, the licensee did not keep furnishings of the home in a safe condition. Evidence - The diaper changing mat observed being used, was still in the plastic wrapping it came in. The directions on the mat state that the covering is to be removed prior to use.

Description: Based on observation, the licensee did not keep all rooms and halls lighted with electric lighting. Evidence - The computer area located past the stairs, has no lighting as the bulb is burned out.

Description: Based on observation, the licensee does not provide equipment of the appropriate size for all of the children. Evidence - Five of the nine children in care are school age and were observed sitting in pre school size chairs at a pre school size table.

Description: Based on observation, the assistant did not provide direct care and supervision to all of the children at all times. Evidence - The assistant was alone with nine children before the licensee arrived. During this time she took one child into the laundry room to change his diaper. She closed the door to the laundry room while she changed his diaper. The other 8 children, ages 13 months - 11 years, were left alone in the play area.

Description: Based on observation, the licensee forced all of the school age children to nap. Evidence - The five school age children in care were all lying down when the inspector arrived at 3:05 P.M. At least three of the school age children were awake.
