54.1-3408 Provider must be MAT certified to administer prescription medication.

Today an unannounced renewal study was conducted beginning at 12:10pm. There were 5 children (14 points) present today with the Provider. Children were finishing lunch, playing with toys and equipment, and napping during this inspection. Five children's records were reviewed today in addition to two adult records. Per a conversation with the Provider, there are no medications on-site for any children enrolled at this time. This inspection, including the exit interview, ended at 1:20pm. Standard #: 22VAC40-111-520-B
Description: Based on direct observation, a crib was being used as a play space for an infant while the Provider was vacuuming. Evidence: While vacuuming the Provider put a 15 month old child in a crib with a mobile play station.

Plan of Correction: I was moving the equipment out of the way while I was vacuuming. I will move it somewhere else other than a crib and not use cribs for a play space.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review, there is no updated criminal history background check for one adult (#2) dated within 3 years of the most recent results. Evidence: The last criminal history background check completed with results is dated 7/29/11 for adult #2.

Plan of Correction: I have already submitted the background checks for renewal and am waiting for the results to be mailed back. I will renew background checks every three years.
