54.1-3408 Provider must be MAT certified to administer prescription medication.

An announced Initial study was conducted today. There are 6 children present (14 points), including one of the provider's own, with 2 adults. 2 adult and 1 children's files were reviewed. See violation notice for missing items. Children were observed playing with a variety of activities. The crib assembled for use has a manufacture date of 1524 (YYWW). Plan of Correction: I will switch to using the model DSS forms.

Description: Based upon a review of files, children's files do not contain written acknowledgement of all required policies and procedures. Evidence: 1 of 1 files reviewed was missing required policies and procedures. Missing information includes but is not limited to: DSS web site address, custodial parent's right's information, medication policy, liability insurance declaration and check in and out procedures.
