Unannounced monitoring inspection was conducted beginning at 10:15 and ending at 11:30. There were 5 children present ages 3 months, 18 months, 2,2,2. Children were observed playing with toys, doing puzzles and looking at books. Reviewed 5 childrens files - complete. Reviewed two adult files - see Violation notice. Description: Based on a review of family files, it was determined that one family member did not have a TB test. Evidence: Adult #2 does not have a TB test.

Plan of Correction: The provider will assure that he has a TB test.

Standard #: 22VAC40-111-180-A
Description: Based on a review of family files it was determined that 1 person did not have a repeat TB test. Evidence: Adult #2 has a TB test that expired in February 2016.

Plan of Correction: The provider will have a TB test.

Standard #: 22VAC40-191-50-A-1-a
Description: Based on a review of Family files, it was determined that 1 family member does not have a sworn statement on file. Evidence: Family member #1 does not have a sworn statement.

Plan of Correction: The provider will assure that the Sworn Statement is completed and signed.
