22VAC40-191 Background Checks for Child Welfare Agencies
Today an unannounced renewal inspection was conducted beginning at 10am. There were 6 children (19 points) present today with the Provider and two assistant caregivers. Children were observed playing at various stations including an indoor slide, blocks, interactive learning equipment, and playing outside. Six children's records and adult records were reviewed today. There are no medications on-site for any children enrolled. This inspection, including the exit interview, ended at 11:15am. Standard #: 22VAC40-111-60-B Description: Based on record review, two children's records (#4 and 6) are missing complete required documentation. Evidence: Child #4 is missing documentation of a physical exam within 30 days of their start date (4/13/15). Child #6 is missing documentation that parents have annually reviewed emergency information. The last documented update was 6/3/14. Plan of Correction: Child #4 just went to the doctor and I'm expecting the parents to give me a copy of the physical any day. I will make sure the record for child #6 is reviewed and signed by the parents. We will annually update all files.
