Technical Assistance:
An exit interview was conducted to discuss the results of today's inspection.

Comments:
An unannounced monitoring inspection was conducted today, April 22, 2016 to determine the provider's compliance with licensing standards. In attendance were 6 children totaling 14 points being supervised by the provider. Children were observed playing computer games and interacting with the provider. Departure procedures were observed as parents signed out their child for the evening. Today's snack consisted of crackers, orange slices and milk. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. The children's restroom was inspected with the required supplies present. All rooms of the home used by children were inspected with no hazards found.

The family day home had the required postings displayed on the bulletin board upon entering the home. The first aid kit, fire extinguisher, and emergency supplies were inspected with the required items found. Two children's and one household member's records were inspected with the one violation found. The following was also inspected: fire drill log, shelter-in-place drill, parent agreements, and other pertinent information.

Standard #: 22VAC40-111-180-A
Description: Based on the inspection of provider records, the required TB test has expired.

Evidence:
During the inspection of one household member's record, it was discovered the TB test/screening dated 4/14/14, expired on 4/14/16. Licensing standards require a TB test or screening to be updated every two years from the date of the initial screening.

Plan of Correction: Per provider: I will have an updated TB test or screening by 4/29/16.
