An unannounced monitoring inspection was conducted on April 28, 2016 from approximately 9:30 am to 11:20 am. Provider and assistant were present with nine children. Children were observed playing freely in the large childcare room and baby room. Licensing Inspector reviewed standards related to supervision, activities, equipment, supplies, nutrition, emergency evacuation and shelter-in-place drill documentation, children's records, caregiver records, household member's record, emergency supplies and required postings. Violations were reviewed with and verified by provider during the exit interview conducted on this date.

Standard #: 22VAC40-111-180-A
Description: Based on review, provider did not ensure each caregiver and adult household member has an updated Tuberculosis test/screening every 2 years. Evidence: On April 28, 2016, the following was found during record review: 1. The record for caregiver #1 has a most recent TB test dated April 21, 2014. 2. The record for household member #1 has a most recent TB test dated April 21, 2014.

Plan of Correction: Individuals will complete the questionnaire for the screening.

Standard #: 22VAC40-111-210-A
Description: Based on review, provider did not ensure caregivers complete a minimum of 16 hours of training annually. Evidence: On April 28, 2016, the record for caregiver #1 has documentation of completing 9 hours of training in 2015.

Plan of Correction: Will do a training online and attend a full workshop to add to current trainings already completed this year.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on review, provider did not ensure household member background checks are updated every 3 years. Evidence: On April 28, 2016, the record for household member #1 has a most recent criminal history check dated March 13, 2013.

Plan of Correction: Individual's signature will be notarized on Saturday and mailed.
