Today an unannounced renewal visit was conducted with the Provider. This visit took place in the morning between 9:15-11:30 there were 7 children with the Provider and 2 assistants for a total of 28 points within the supervision guidelines. The children were observed having morning art, snack, playing with blocks, lesson and in transition for nap and lunch time. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, menu, attendance, and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-920 Description: Based on observation of the menu, the menu posted did not reflect the current one-week period. Evidence: The menu posted was for the month of April. Provider did not have a menu for the week of May 2nd-6th. Plan of Correction: Provider stated that she will add the menu immediately. She forgot to post it.
Standard #: 22VAC40-111-120-B Description: Based on review of the provider records, one record did not include documentation of two or more references were checked and verified prior to employment. Evidence: Staff #3's record. Plan of Correction: Provider stated that she will contact them today and request information.
Standard #: 22VAC40-111-240-A Description: Based on observation of the physical plant, the following areas outside were found not in a clean, safe and operable condition. Evidence: Upon arrival, the gate to enter the family day home was found warp/broken and unable to open. Plan of Correction: Provider stated that she will have it fixed immediately.
