Technical Assistance:
Discussed with provider: potential mattress recall for IKEA and waterproof coverings, gate requirement for the playground stairs.

An announced Initial inspection was conducted today. There are 4 children present (8 points) with 3 adults. 5 adult and 3 children's files were reviewed. See violation notice for missing information. The cribs assembled for use have acceptable manufacture dates of 1230 (YYWW). Children were observed dancing, working puzzles, and enjoying free play time, washing hands. Standard #: 22VAC40-111-800-A
Description: The written emergency preparedness plan completed by the provider does not have the same relocation address as the plan notification in the children's files. There are multiple addresses listed.

Plan of Correction: I will update the plan and the children's folders to reflect the same address.

Standard #: 22VAC40-111-60-B
Description: Based upon a review of files, children's files are incomplete. Evidence: 3 of 3 files reviewed were missing: complete emergency contact information. (Children 1,2,3)

Plan of Correction: Parents will be asked to complete all new forms.

Standard #: 22VAC40-111-70-A
Description: Based upon a review of files, parents have not been notified on writing prior to the first day of attendance of all required policies/ procedures. Evidence: 3 of 3 files reviewed were missing documentation of all required polices/procedures. Missing information includes but is not limited to: check in/out procedures, custodial parents rights statement, the DSS web site address.

Plan of Correction: I will switch to using the model DSS forms that includes all the information.

Standard #: 22VAC40-111-320-A
Description: Based upon direct observation, small appliances were plugged in while not in use. Evidence: Two hair dryers were plugged in, in upstairs bathrooms.

Plan of Correction: Unplugged during the inspection. I will check every morning to make sure they are unplugged.
