Comments:
An unannounced monitoring inspection was conducted beginning at 10:55 a.m. and ending at 12:00 p.m. with the provider and assistant. There were six chidlren in care ranging in age from nine months to four years of age for a total of sixteen points. Children were watching an age appropriate television program at the beginning of the inspection. After television time, children were observed having free play. One child was observed practicing handwriting and writing his name. The childcare area and nap room were observed. Three household member records, two provider records and three children's records were reviewed. The provider is Medication Administration Training (MAT) certified but there are no medications on site. Standard #: 22VAC40-111-690-D
Description: Based on observation, the diapering surface was absorbent. Evidence: One diapering pad had cracks in the nonabsorbent material on either side. The second diapering pad had an absorbent material around the outside edges of the pad.

Plan of Correction: A new diapering pad will be purchased.
