Comments:
An unannounced monitoring inspection was conducted beginning at 10:00 a.m. and ending at 11:45 a.m. with the provider. There were three children in care ranging in age from eight months of age to four years old for a total of eight points. Children were observed reading books and watching an age appropriate television program. The childcare area was observed. One newly enrolled child's record was reviewed. Violations:
Standard #: 22VAC40-111-80-A
Description: Based on review of documentation, the provider failed to obtain proof of age of identity within seven days. Evidence: Child A, did not have proof of age and identity in the record.

Plan of Correction: The parent has been repeatedly asked for this documentation. The parent was reminded again this morning at drop off. By the end of the day tomorrow, this will be documented.

Standard #: 22VAC40-111-530-F
Description: Based on observation, there soft bedding and a pillow was observed in a crib. Evidence: There is an eight month old child in care and one crib which was observed with soft bedding (filled comforter, large blanket) and a pillow in a crib.

Plan of Correction: The bedding and pillow were removed during the inspection and won't be used in the crib in the future.
