22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced monitoring study was conducted with the provider and four children for a total of seven points. The ages of the children range from two years-old through five years-old. The children were having free play during the inspection. Two household records and two children's record were reviewed. The provider is current in First Aid and CPR. Start time: 9:00 a.m. End time: 10:45 a.m.
Standard #: 22VAC40-111-60-A Description: Based on a direct review of two children's records, the child's enrollment form was not updated annually for one record. Evidence: Child A's form should have been updated on /5/4/2016. Plan of Correction: The provider will have the parent update and sign the form today.
Standard #: 22VAC40-111-60-B Description: Based on a direct review of two children's records, one record was missing information. Evidence: Child A's was missing the first day of attendance and the policy number for the child's medical information. Plan of Correction: The provider will have the missing information corrected by today.
