An unannounced renewal study was conducted from 10:45 a.m.-12:15 p.m. with the provider and assistant. Ten children, ages 10 months, 17 months, 23 months, 2 years, 2 years, 2 years, 2 years, 3 years, 3 years old, and 5 years old were in the care of two caregivers for a total of 23 points. Children were observed listening to a book with puppet, building train tracks and building with blocks, drawing and coloring, and preparing for lunch. Lunch served today is chicken nuggets, rice, green beans, apple slices, and milk. The first aid kit, flashlight, and weatherband radio were present and complete. The emergency escape drill was 5/3/16. The provider and assistant are both current in first aid and CPR. Three children's records were reviewed and in compliance. Description: Based on review of two household members' records, the provider did not have a tuberculosis (tb) test or screening for each adult household member. Evidence: Household Member A did not have documentation of a tb test or screening in his file.

Plan of Correction: Household Member A will get it as soon as possible.
