Licensing Inspector arrived at 11:30 AM and departed at 12:45 PM. At the time of the inspection, there were eight children (ages infant through preschool), for a total of 22 points, in care with two caregivers. Children were observed watching television and playing in centers. The sample size was four caregiver/household records and five children's records.
Standard #: 22VAC40-111-60-B Description: Based on a review of five children's records, it was determined that the provider did not ensure each child's record contained all the required information. Evidence: Child #5 (date of enrollment 11/16/15), who was in care at the time of the inspection, had only one of the two required persons to contact in case of an emergency. Plan of Correction: Provider will obtain information from parent to complete the record.
Standard #: 22VAC40-111-180-A Description: Based on a review of four caregiver and household member's records, it was determined that the provider did not ensure that a current report of tuberculosis (Tb) screening form was obtained every two years. Evidence: Caregiver / household member #1 and #3 had Tb screenings on file that were dated 10/20/13 and were, therefore, were over two years old. Plan of Correction: Provider will ensure that Tb screenings are updated.
