Comments:
An unannounced monitoring inspection was conducted on 07/14/2016 from 9:20am - 12:00pm. During the inspection there were nine children ages five months to three years old (27 points) in care with two caregivers. A tour of the home was conducted and children were observed playing with a variety of toys, playing outside, and eating lunch. Records were reviewed for four children and three caregiver/household members. Emergency plan and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on a review of four children's records, it was determined that the provider did not ensure they maintain and keep at the family day home a complete record for each child enrolled that contains all required information. Evidence: 1. The record for child #1, present during the inspection, did not contain the address and phone number for the father's place of employment, the address of the second person to contact in case of emergency, and did not indicate if the child had previously attended child care. 2. The record for child #2, present during the inspection, did not contain documentation of a second person to contact in case of emergency. 3. The record for child #3, present during the inspection, did not contain documentation that an annual review of the child's record had been completed to ensure that all information was correct. The most recent did documented in the record was 4/15/15. 4. The record for child #4, present during the inspection, did not contain documentation for the date of enrollment. 5. The provider reviewed the records for child #1, #2, #3 and #4, and confirmed the records were not complete.

Plan of Correction: All missing information will be request from the parents.
