Technical Assistance:
The inspector and the home provider discussed that the annual training hours need to be completed by August 1st 2016. Currently the provider has 14.5 hours and is required to have 16 hours during the course of 1 year.

An unannounced renewal inspection was conducted on July 22nd 2016 between 8:30am and 10:20am. There were 5 children (12 points) in the direct care of a provider and 2 assistants. Children were observed playing, sharing, eating breakfast (oatmeal with fruit and water). Children have artwork displayed throughout the family day home space. A sampling of children's records, household records, and staffing records were reviewed. The evacuation drills, shelter-in-place drills, the emergency supplies, supervision, the physical plant, and first aid kit were also inspected and observed. Standard #: 22VAC40-111-940-B
Description: Based on observation the family day home did not have an operable thermometer available to monitor refrigerator and freezer temperatures. Evidence: There was not a thermometer in the refrigerator that is located on the lower level of the home that is used for the day care.

Plan of Correction: Assuming that the refrigerator was new I thought that it didn't need it. I will get a thermometer.

Standard #: 22VAC40-111-50-A
Description: Based on observation and interview, the provider did not have a written record of children in attendance each day. Evidence: The provider stated that she does not have an attendance record because she was told by a previous inspector that it was no longer required.

Plan of Correction: Starting August 1st I will start taking attendance.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 3 of 3 children's records were incomplete. Evidence: Child #1 record did not contain - 1. a 2nd emergency contact 2. the medical insurance policy number. Child #2 record did not contain - 1. the sex of the child 2. 2 emergency contacts 3. the parents' work address and telephone numbers. 4. 1st date of attendance. 5. medical insurance provider and policy number 6. authorization for medical care. 7. authorization to pick up information. Child #3 record did not contain 1. Documentation of immunization

Plan of Correction: I will contact the parents and ask for them to update the forms.
