Discussed with provider that all parent forms must be dated by the parents. Discussed practicing a shelter in place drill and documenting on the fire drill form. Provided copies of field trip forms, injury form, and diaper cream form.
The following inspection took place between the hours of 12:00 pm through 1:45 pm. Today, I met with the Provider on site at the Family Day Home (FDH). At the start of this unannounced monitoring inspection there were a total of 7 children (10 points) with provider. A total of 3 children's files and 3 staff/household member's files were reviewed. The children were engaged in the following: toy play, coloring, puzzles, bathroom breaks, hand washing. The violations were reviewed with the provider. Standard #: 22VAC40-111-830-B Description: Based on record review and provider interview, a shelter- in place has not be practiced a minimum of twice per year. Evidence: There is no documentation of a shelter in place drill. Plan of Correction: I will practice shelter and write on fire drill form.
Standard #: 22VAC40-111-140-B-4 Description: Based on record review and provider interview, provider shall have current certification in first aid. Evidence: There is no documentation on the current Heartsaver CPR card to include certification in first aid. Plan of Correction: Provider said that she took first aid, the provider called to get information about the first aid card. The provider will follow up with Heartsaver to get a copy of her card.
