Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Child #1 and Child #2, enrolled 6/27/2016, did not have documentation of a physical examination.

Plan of Correction: Parents were notified.

Standard #: 22VAC40-111-50-A
Description: Based on record review, the provider did not keep a written record of children in attendance each day. Evidence: The last date that attendance was recorded was 7/28/2016.

Plan of Correction: A written copy will be kept every day.

Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider did not maintain complete records for all children in care. Evidence: Child #1 and Child #2 were missing the street address for 1 of 2 required designated persons to contact in case of an emergency if the parent cannot be reached.

Plan of Correction: Parents were notified. Waiting for information.

Description: Based on record review, the provider did not provide all required information to parents. Evidence: There was no signed acknowledgement form for Child #1 and Child #2 informing the parents of the provider's liability insurance policy.

Standard #: 22VAC40-111-80-A
Description: Based on record review, the provider did not verify the age or identity of all children within seven business days of the child's first day of attendance. Evidence: Child #1 and Child #2 was missing evidence of an age or identification verification.

Plan of Correction: Birth certificates obtained.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider did not obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations prior to the first date of attendance. Evidence: Child #1 and Child #2 were missing a record of immunization.

Plan of Correction: Parents were notified.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain a current report of Tuberculosis screening every two years from the date of the first screening. Evidence: 1. Staff #1's last dated Tuberculosis screening was conducted 6/3/2014.

Plan of Correction: Although I attended the clinic, another appointment will be attempted as advised.

Standard #: 22VAC40-111-240-A
Description: Based on observation, did not maintain all areas of the home in safe and operable condition. Evidence: 1. The gate leading from the deck area to the yard area used by children had 40 exposed screw heads that could snag skin. 2. The fence on the left side of the play yard had a n 18 inch rusted metal wire with exposed sharp points woven through several links in the chain link fence.

Plan of Correction: 1. Screw heads will be secured. 2. A safety nest will be placed over chain fence.

Standard #: 22VAC40-111-320-A
Description: Based on observation, all small electrical appliances were not unplugged while not in use. Evidence: A small electric kettle was observed plugged into a kitchen outlet.

Plan of Correction: Correction made.

Standard #: 22VAC40-111-480-D
Description: Based on observation, not all equipment used by children was used in accordance with the manufacture's instructions. Evidence: 1. Child #5 was placed in a high chair without the provided safety belt being fastened from approximately 10:45am-11:30am. 2. Child #5 made numerous attempts to exit high chair and on one occasion was able to extend a leg above the tray and over the side of the chair attempting to exit the chair. 3. Child #3 and Child #4 were placed in high chairs without the provided safety belt being fastened from 11:40am-11:55am.

Plan of Correction: Safety belts will be used at all times.
