An unannounced monitoring inspection was conducted on August 23, 2016. There were 4 children present with 1 provider. The total number of points for children in care during the inspection were 6. The children were observed eating lunch, playing freely, and napping. The provider does not administer medications as a part of the family day home. Transportation is not provided by the family day home. The sample size of records reviewed were 3 children's records and 1 provider record and 1 household member record. The designated outdoor play space was observed. The violations from the previous inspection were reviewed and corrected. The inspection began at approximately 10:55 a.m. and ended at 12:45 p.m..
Standard #: 22VAC40-111-60-A Description: Based on documentation review and provider interviews, the provider did not ensure the provider maintained an up-to-date record at the family day home for each enrolled child. Evidence: During the inspection on August 23, 2016, the record for child #3 did not include proof of identity. Plan of Correction: The provider requested document from the parent during inspection.
Standard #: 22VAC40-111-210-A Description: Based on documentation review and provider interviews, the provider failed to obtain 16 clock hours of training annually. Evidence: During the inspection on August 23, 2016, the providers records had documentation of only 8 hours of annual training. The provider confirmed the annual training was maintained from June 2015 to June 2016. Plan of Correction: The provider will continue working on obtaining training.
Standard #: 22VAC40-111-280 Description: Based on observation and provider interviews, the provider did not ensure that potentially poisonous substances, materials and supplies were stored inaccessible to children. Evidence: During the inspection on August 23, 2016, one container of antibacterial hand soap was observed on the bathroom counter labeled "keep out of reach of children" and at least one of the other statements "warning" and "caution". Plan of Correction: The provider removed the hand soap.
