Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and set capacity. Discussion that paperwork must be in place for each new assistant that begins work at the Family Day Home (FDH); with the exceptions to those documents with pre-set grace periods.

The following inspection took place between the hours of 10:30 am through 12:30 pm. Today, I met with the Provider on site at her Family Day Home (FDH) located in Ashburn, Virginia. At the start of this unannounced monitoring inspection there were a total of 10 children (see issued violation notice) (26 points) with 2 caregivers. A total of 10 children?s files and 4 staff/household member?s files were reviewed. The children were engaged in the following: freeplay, circle time, movie time, song, music, dance, bathroom breaks, hand washing, toy play, lunch, and other organized activities. Standard #: 22VAC40-111-120-A
Description: Based on review and interview, 1 of 4 staff/household member's files reviewed indicated that the Provider did not maintain a record for each caregiver. Evidence - 1. On the date of inspection (09/07/2016), the record for Member #2 was not available for review. 2. The Provider stated (during an interview on the date of inspection) that Member #2 just started this week and in on a trial basis.

Plan of Correction: A file will be created for all caregivers and the necessary information/documentation shall be obtained.

Standard #: 22VAC40-111-40-A
Description: Based on observation, the Provider did not ensure that the total number of children receiving care at any one time does not exceed the maximum licensed capacity of the home. Evidence - 1. On the date of inspection (09/07/2016, starting at 10:30 am) there were a total of 10 children in care. 2. The total set capacity for the FDH is 9. 3. None of the children in attendance were residents in the household.

Plan of Correction: The Family Day Home (FDH) shall reduce to set capacity immediately. A request for a formal pre-application conference to file for a minor special exception with the Loudoun County - Department of Planning & Zoning has been set for Tuesday, September 13, 2016. This form was dated - 08/11/2016. Once this exception has been approved, such documentation shall be submitted to the Division of Licensing in order to make any and all needed modifications to current license.
