Comments:
An unannounced monitoring inspection was conducted from 2:45 p.m.-4:00 p.m. with the provider. Four children, ages 16 months, 21 months, 3 years, and 4 years old, were in the care of two caregivers for a total of ten points. Children were observed building with Duplo blocks with the provider, cleaning up, singing and dancing, and eating afternoon snack. Afternoon snack today is graham crackers and apples. Required information was posted. The last emergency escape drill was 9/8/16. One medication was reviewed. One child's and one household member's records were reviewed and complete. Violations:
Standard #: 22VAC40-111-700-A
Description: Based on review of one medication, the provider did not have written authorization from the parent to administer prescription medication. Evidence: The parent's written authorization for Child A's, age 10, Epipen Jr. expired 3/26/16.

Plan of Correction: The provider stated that she already addressed this with the parent, and the parent will be bringing the long term medication authorization form at pick-up today.

Standard #: 22VAC40-111-50-A
Description: Based on observation, the provider did not keep an accurate written record of children in attendance. Evidence: The attendance record for Wednesday, September 21, 2016 reflected ten children's names. Four children were physically present.

Plan of Correction: The provider corrected the attendance during the inspection.
