Comments:
Today an unannounced monitoring visit was conducted with the Provider. This visit took place in the morning between 9:15-11. There were 4 children present with the Provider for a total of 15 points within the supervision guidelines. The children were observed having morning free play, tummy time, learning colors, numbers, playing with puzzles and in transitions for lunch. Lunch today consisted of spaghetti, fruit and milk. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, attendance, menu and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-111-920
Description: Based on a review of the menu, the menu posted did not reflect the current one-week period. Evidence: The menu posted was dated September however it did not include a date.

Plan of Correction: Provider stated that she will include a date.

Standard #: 22VAC40-111-50-A
Description: Based on observation and review of the attendance, the provider did not have documentation of a record of children in attendance. Evidence: Upon arrival, 4 children were present however the provider did not have a written record of children in attendance.

Plan of Correction: Provider stated that she have not completed the attendance for today.

Standard #: 22VAC40-111-60-B
Description: Based on review of the children records, one record did not include documentation of two designated emergency contact person's name, address and phone number. Evidence: Child #3's record did not include one of the designated emergency contact person's phone number.

Plan of Correction: Provider stated that she will request the information from the parent today.

Standard #: 22VAC40-111-340-C
Description: Based on observation of the physical plant, the bathroom did not have paper towels. Evidence: In the bathroom located near kitchen, used by the childcare children a towel was found in the bathroom and no paper towels were available for the children to dry their hands.

Plan of Correction: Provider immediately removed the towel and added paper towels to the bathroom.
