An unannounced renewal inspection was completed on 9/28/2016. There were two children in care with the provider supervising at a total of four points. Five children's records, the provider's record and one household member's record were reviewed. The inspector arrived at the provider's home at 8:40 am and departed at 11:30 am. The children were observed during free play times, during morning snack, during a pre-writing activity for two older children, and during large group calendar/weather. There was discussion with the provider about the following: outside time, schedules and new process for subsidy.

Standard #: 22VAC40-111-830-C
Description: Based on review and interviews with the licensee, the evacuation drills were not documented. Evidence: 1. The inspector found that the evacuation drill for August 2016 was not documented. The licensee stated that it was completed and just not documented.

Plan of Correction: The licensee will document as the drills happen.

Standard #: 22VAC40-111-240-A
Description: Based on observation of the playground area, the licensee failed to ensure that all areas and furnishings where maintained in a clean, safe and operable condition. Evidence: 1. The inspector found a riding car that the back end was cracked and had sharp pieces exposed.

Plan of Correction: The licensee place the car over the fence and will discard the car ASAP.

Standard #: 22VAC40-111-320-G
Description: Based on record review and interview with the provider, the provider failed to ensure that a wood burning stove in the home was inspected annually. Evidence: 1. The last documentation of an inspection of the wood burning stove and associated chimney was October 2010.

Plan of Correction: The provider will get an inspection done for the wood burning stove and chimney. For future, the provider will ensure that this is done annually and documentation is maintained.

Standard #: 63.2-1809.1-A
Description: Based on review of five children's records, the center failed to ensure that all paper work was completed by the parent. Evidence: 1. The inspector found that child # 3 did not have a signed copy of the liability insurance from the parent.

Plan of Correction: The license will have the parent to sign the copy and will make sure that the parents has a copy.
