Comments:
An announced Initial inspection was conducted today. There are three children for a total of 10 points with one adult. One adult and three children's files were reviewed. See violation notice for missing items. There are four cribs assembled for use with acceptable manufacture dates. Children were observed during diaper changing and enjoying playtime. Due to case load adjustments your next inspection will be with a different inspector. All inspectors carry ID badges for security purposes. LI was present from 9:30 AM - 11:00 AM.

Standard #: 22VAC40-111-920
Description: Based on observation menu was not posted in home.

Plan of Correction: I will post the menu.

Standard #: 22VAC40-111-960-E
Description: Based on observation bottles were not labeled and dated.

Plan of Correction: I will label the bottles.

Standard #: 22VAC40-111-60-B
Description: Based upon review of files, children's files are incomplete. Evidence: 2 of 3 files reviewed were missing: Father's infomration and emergency contact addresses. (Children 2,3)

Plan of Correction: I will ask mother to provide information.

Standard #: 22VAC40-111-90-C
Description: Based upon a review of files, 1 child's immunization form is incomplete. Evidence: There is no immunization record available for child #3.

Plan of Correction: I will ask the mother to provide information.

Standard #: 22VAC40-111-690-E
Description: Based on observation a diaper was not disposed of in a foot operated trash can.

Plan of Correction: A foot operated can from the bathroom was relocated to the diaper changing area.
