Comments:
Today an unannounced monitoring inspection was conducted from 11:30am-1:25am. There were 5 children( totaling 14 points) directly supervised by 2 staff. The home, 1 staff records, 2 children?s records, evacuation drills, and policies were inspected. There were no medications on site today. Children were observed playing freely and eating lunch. Hand washing procedures were also observed. There was a sufficient supply of materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-780-B
Description: Based on record review, 1 of 2 children's files did not document the annual review of the child's emergency contact information. Evidence: 1. Child #2's emergency information was last review on 11/3/2015.

Plan of Correction: I will have parent review information today.

Standard #: 22VAC40-111-50-A
Description: Based on direct observation the family day home did not have a written record of the children in attendance. Evidence: 1. There was no attendance sheet available for review.

Plan of Correction: I will get it done today.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 1 of 2 children's records were incomplete. Evidence: 1. Child #1 was missing both parents' work phone number. 2. Child #2 was missing the name and policy number of the child's medical insurance.

Plan of Correction: I will get the information today.

Standard #: 22VAC40-111-140-B-4
Description: Based on record review, there was no provider present with current certification in first aid. Evidence: 1. Provider #1's first aid certification expired on 3/29/16. 2. Provider #2' first aid certification expired on 4/24/16.

Plan of Correction: We will get certified.
