Comments:
An unannounced monitoring inspection was conducted from 12:15pm to 1:30pm. At the time of inspectors arrival there were 9 children, ages 2 mos to 4 years, for a total of 26 points, in care with the provider and one assistant provider. The records sample size consisted of 4 children's records and 3 provider / substitute/ household member records. Children were observed playing indoors, and transitioning between activities. The provider stated that no medication is administered to children while in care and there have been no injuries since the last inspection. Areas of noncompliance are identified in the violation notice and were discussed with the provider during the meeting at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review it was determined the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: The record for child #2(enrolled 5/4/16) did not have documentation of a physical exam.

Plan of Correction: Provider will request physical from parent at pick-up.

Standard #: 22VAC40-111-60-B
Description: Based on record review it was determined the provider did not ensure that each child's record contained all of the required components. Evidence: 1. The record for child #1(enrolled 9/7/16) did not contain: -Insurance policy number -Address for 1 of the 2 emergency contacts -Verification of identity and age 2. The record for child #2 did not contain: -Addresses for the 2 emergency contacts 3. The record for child #3(enrolled 1/4/16) did not contain: -Insurance policy information -Addresses for the 2 emergency contacts -Authorized persons to pick up 4. The record for child #4(enrolled 6/8/15) did not contain: -Annual review and update(if needed) of record

Plan of Correction: Provider will have parents provide the missing information at pick up.

Standard #: 22VAC40-111-140-B-3
Description: Based on record review it was determined the provider did not maintain current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care, from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current CPR certification issued within the past two years by a community college, a hospital, a rescue squad, or a fire department. Evidence: The provider's and assistant's CPR certifications had expired 9/6/16.

Plan of Correction: CPR training has been scheduled for 11/11/16.

Standard #: 22VAC40-111-140-B-4
Description: Based on record review it was determined the provider did not have current certification in first aid from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current first aid certification issued within the past three years by a community college, a hospital, a rescue squad, or a fire department. Evidence: Provider's and assistant's first aid certifications had expired 9/6/16.

Plan of Correction: Provider and assistant are scheduled to attend first aid training on 11/11/16.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record check it was determined the provider did not ensure that an updated criminal history record was received before three years since the dates of the most recent criminal history record check. Evidence: The provider did not have current criminal history record check. The record check available was dated 6/26/13.

Plan of Correction: Provider will contact the State Police regarding criminal record check.
