Technical Assistance:
An exit interview was conducted with the provider to discuss the results of today's inspection. Consultation was given regarding the fire extinguisher and the required Preservice Training.

An unannounced renewal inspection was conducted today, November 4, 2016 to determine the provider's compliance with licensing standards. During today's inspection one child was present totaling 3 points being supervised by the provider. The child was observed asleep on the appropriate bedding. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. All rooms of the home including the upstairs were inspected with one hazard found. The restroom used by children were inspected with the correct equipment and supplies present. The outdoor play areas and equipment were inspected and found to be in operable and safe condition. The koi pond was observed to be surrounded by a fence and locked to prevent entry.

The family day home had the required postings displayed on the bulletin board and wall upon entering the home. The first aid kit, fire extinguisher, and emergency supplies were inspected. Two children's and all provider records were inspected with the required information found. The following was also inspected: fire drill log, shelter-in-place drills, emergency preparedness plan, parent agreements, and other pertinent information. Standard #: 22VAC40-111-940-A
Description: Based on the inspection of the refrigerator, the required refrigerator temperature was not maintained. Evidence: During the inspection of the refrigerator, it was discovered the temperature exceeded the maximum requirement of 40 degrees. Samples taken measured 43 degrees Fahrenheit.

Plan of Correction: Corrected on site.

Description: During the inspection of the bathroom, poisonous/ hazardous substances were found accessible.

During the inspection of the children's bathroom, poisonous/hazardous substances were found on the rear base of the toilet in a basket accessible to children. Items observed were bath salts, fluid lighter, etc.).
