54.1-3408 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
Discussion of expiration of reviewing expiration dates on topical products to ensure that those items do not expire. Discussion on including the name of the topical ointment on each consent form.

Comments:
Unannounced monitoring inspection was conducted from 8:30am to 10am. Six children present during inspection with two staff with a total of 14 points. One household member present during inspection. Observation of transition from before school care of two school aged children to the bus stop. Children observed finishing breakfast and playtime. Some violations found today. Violations:
Standard #: 22VAC40-111-700-B
Description: Based on review of medications, one medication was found without written authorization. Evidence: Ibuprofen for child #1 was maintained without a written authorization for administration from the parent.

Plan of Correction: Child #1 no longer is required Ibuprofen, provider will return medication to the parent.

Standard #: 22VAC40-111-730-D
Description: Based on walk thru of physical plan, one medication was found unlocked. Evidence: Ibuprofen for child #1 was found unlocked in the diaper changing area.

Plan of Correction: Medication was removed and locked immediately.

Standard #: 22VAC40-111-750-A
Description: Based on walk thru of physical plan, topicals were not labeled. Evidence: Diaper creams were found above diaper changing area not labeled.

Plan of Correction: Provider will label all topical ointments/creams immediately.

Standard #: 22VAC40-111-110-B
Description: Based on review of children's files, one immunization record was not maintained Evidence: Child #1's immunizations were not obtained as required to substantiate the child was adequately immunized.

Plan of Correction: Will obtain current record of immunizations from child #1's parent.

Standard #: 22VAC40-111-560-A
Description: Based on observation of physical plant, child #2 was not supervised while sleeping in a swing. Evidence: Child #2 was found sleeping in a swing in the crib room down the hall, this room was out of direct supervision from the caregiver.

Plan of Correction: Caregiver will no longer use the swing located in the other room but will use the swing located in the general play area to allow for direct supervision at all times.
